Some vascular pathology in patients with headache and arterial hypertension

Abstract

In order to determine the structure of vascular pathology in patients with headache (HA) and increased blood pressure (BP) (35 patients), ultrasound duplex scanning of the neck vessels (USDS) and transcranial duplex scanning of the head vessels (TCDS) were performed, and functional tests with the abduction of the upper extremities and measurement of arterial and venous pressure were conducted.

Significant changes in the characteristics of the right internal jugular vein (IJV) in patients with headaches, dilatation of the right vertebral vein (VV) were revealed. In people without headaches, with comparable sizes of the left veins, the rate of venous outflow was significantly higher. In the arterial bed, significant differences in time‑averaged blood flow velocity were found in both common carotid arteris (CCA) and in the right VA, with a predominance of indicators in the control group. In the main group, valvular insufficiency of the right IVF was 2.33 times more common, and left IVF was 7 times more common than in the control group. In patients with cephalgia, the left CCA moved away from the brachiocephalic trunk or its angle 3.66 times more often than in the control group; thoracic outlet syndrome (TOS) was detected 4 times more often. According to the results of TCDS, changes in the right SMA and PMA are more significant than on the left: an increase in peripheral resistance was more common than in the control group (PI 5.3 times, RI 3.43 times). Significant changes in blood flow in the direct sine were detected in the venous intracranial system in patients with hypertension.

Sofya I Mizgireva is a candidate of the Department of Cardiology, Ultrasound and Functional Diagnostics with a course in pediatric Cardiology of the Bakulev Federal State Budgetary Institution of the Russian Academy of Medical Sciences. She is the head of ultrasound department of multidisciplinary Medical Center “MEDIN" in Yaroslavl, Russian Federation. Also she is the author of publications.
