Title: Fenestrated ASD device closure in elderly woman with a very large ASD

Abstract

A 53-year-old female patient with atrial fibrillation and atrial septal defect of 42mm size (TEE 3D Sized) with baseline LVEDP 10mmHg    and PASP 28-30mmHg underwent TEE guided ASD device closure. 44mm custom made fenestrated ASD device (Lifetech Scientific, PRC) was successfully deployed without periprocedural complications. Before release of device LVEDP rose to 14mmHg and hence device was successfully deployed and patient remained stable after the procedure.

Custom made fenestrated ASD devices are available and are of great value in ASD with significant left to right shunts with restrictive LV physiology or significant pulmonary artery hypertension. This case illustrates that ASD device closure in elderly in above said scenarios aid in improvement of functional class, right atrial and ventricular reverse remodeling by reducing the shunt fraction yet allowing ventricles to vent on either direction thus preventing hypertensive crisis and worsening RV function.

Biography

Sriram Veeraraghavan, is currently affiliated to cardiology department of SRM medical college and hospital, Chengalpattu, Tamil Nadu, India. He completed his DM (cardiology) at the age of 34 years from Sri Jayadeva Institute of Cardiology, Bangalore, India. He has over 30 publications in various reputed international cardiology journals. He is also serving as an editorial board member of several reputed journals.
