Title: Effect of age, menopause stage and blood pressure on arterial stiffness in postmenopausal women

Abstract

Context: Arterial stiffness (AS) is an index of vascular health and has been considered a crucial independent predictive risk factor for cardiovascular disease. Postmenopausal women exhibit heightened AS compared to their premenopausal counterparts, however the factors explaining these higher values are not well understood. 

Objective: The aim of this study was to investigate the association between AS, age and blood pressure (BP) among postmenopausal women. 

Methods: this is a cross sectional study with 65 women (56.36±6.72 years old; 27.23+3.66 kg/m2; early postmenopause (EP): 35; late postmenopause (LP): 30). Most women experienced a natural menopause (n=57) and did not use hormone therapy (n=52). BP was assessed using the OMRON 705IT. Hypertension was defined as systolic (SBP) and diastolic blood pressure (DBP) >120/80 mmHg. The AS was determined through carotid-femoral Pulse Wave Velocity (cfPWV) with the SphygmoCor® AtCor device, and a normal cfPWV was set as <10m/s. A cluster analyses was used to split two groups based on age. The Mann-Whitney test was employed to compare values from independent groups, controlling for menopause stage. Stepwise multiple regression analysis was conducted. 

Results: In the context of elevated SBP, a substantial increase in AS was observed in comparison to individuals with normal SBP levels (+1.19 m/s; p<0.01). Similarly, this trend was evident among older women (Cluster 2, +1.05 m/s; p=0.02). Notably, women characterized by both high SBP, and advanced age exhibited even higher AS values, with increases of +1.49 m/s (p<0.01) and +1.05 m/s (p=0.02), respectively. Furthermore, the prevalence of cfPWV>10m/s was 15.4% within the study cohort. 

Conclusions: Our findings indicate that, regardless of menopausal stage, both age and SBP are influential factors contributing to increased AS.
