Title: Case report : Management of patent ductus arteriosus with pulmonal valve atresia without ventricular septum defect in premature infant

Abstract

Background: Pulmonary Atresia with Intact Ventricular Septum is a rare heart anomaly that can be congenital or acquired. This condition prevents blood flow from being pumped to the Mean Pulmonary Artery (MPA). Patent Ductus Arteriosus (PDA) is a common form of a persistent duct, used in this case as an abnormal pathway for blood flow from the heart to the lungs. Case Report: A 3-day-old premature infant weighing 1.5 kg exhibited cyanosis and increased respiratory effort on day 3 of care based on physical examinations and supporting tests such as laboratory work, EKG, echocardiography, chest X-ray (babygram), and angiography. Diagnosis revealed severe respiratory distress syndrome (HMD Grade II), Sepsis Neonatorum New Onset, PDA with Pulmonary Atresia. Treatment included intravenous injections of Ampicillin, Gentamicin, Aminophylline, Omeprazole, Alprostadil injection and PDA Stenting measures. Discussion: PDA with Pulmonary Atresia is a rare heart anomaly that can be congenital or acquired. This condition can cause a medical emergency in neonates due to the inability to pump blood into the MPA, resulting in blood flow to the lungs only through the PDA, known as a Ductus Dependent abnormality. It is categorized as a Critical Congenital Heart Disease (CCHD). 

Summary: A case is presented of a 3-day-old premature female infant weighing 1.5 kg. Based on the history, physical examination, and supportive tests, the patient was diagnosed with severe respiratory distress syndrome (HMD Grade II), Sepsis Neonatorum New Onset, PDA with Pulmonary Atresia. Treatment involved intravenous injections of Ampicillin, Gentamicin, Aminophylline  Omeprazole, Alprostadil injection and patien have a clinical improvement after the PDA Stenting procedure was carried out.
