Prognostic value of ST segment deviation in lead aVR in ECG in acute myocardial infarction

Abstract

Background: The 12-lead electrocardiogram is a crucial tool in diagnosis and risk stratification of acute coronary syndrome and coronary artery disease. In the current study, we evaluate the importance of lead aVR in diagnosing Myocardial Infarction among Indian patients.


Methods: History, physical examination, electrocardiogram for detecting ST deviation and Doppler echocardiography for estimating Left Ventricular Ejection Fraction rate was conducted in MI patients. 30-day mortality and arrhythmia changes were assessed at follow-up.


Results: Among 110 patients in the study, no significant prognostic significance of aVR lead in detecting MI was found in the current study. In the sub-analysis conducted between ST isoelectric (n=55) and ST elevation (n=27) and between ST isoelectric and ST depression (n=28) also found no significant prognostic value. However, an increase in risk of complications such as Ventricular Tachycardia among patients with ST depression was observed (p=0.0045) when compared to ST isoelectric patients.


Conclusion: Having prior knowledge ST deviation changes in aVR lead might provide more information on the risk of post-surgery complications such as Ventricular Tachycardia.
