First report of simultaneous endovascular endoprosthetics of the aortic arch and its branches combat injuries injury

Abstract

We have summarized the latest data on endovascular endoprosthetics of the aortic arch and its branches. We plan to present a case of the first unique operation of total endoprosthetics of the aortic arch and its branches in a patient with post-traumatic aneurysm of the aorta and its branches. The report describes in detail the technique of its implementation, highlights the advantages of this method, and possible complications. We used aortic arch endoprosthetics with supra-aortic endodebranching, an effective alternative method for treating patients with thoracic aortic pathology who, for various reasons, are not candidates for traditional prosthetics using artificial circulation. Thanks to this approach, we avoided the need for open surgery on the aorta, which reduced risks and contributed to a faster recovery for the patient. The next day, the patient was already able to move independently, which indicates the minimal invasiveness and effectiveness of the procedure, without any neurological complications. The patient returned to normal life without significant limitations, which is evidence of the success of the endovascular approach. This case is unique, demonstrating the significance and emphasizing the importance of innovation in medicine, as well as the cohesion and professionalism of the team of doctors. We were able to use endovascular methods to treat a complex combat injury, which opens up new possibilities for treating similar cases.

 

Biography

Alexander Vladimirovich Ivanov is the head of the department of X-ray surgical methods of diagnosis and treatment (with an operating theatre and anaesthesiology-resuscitation team) at the 83-bed cardiac surgery center, as well as a doctor specialising in X-ray endovascular diagnosis and treatment. After graduating from medical school, he has been working at the institution since 2003, progressing from intern (2003) to radiologist (2004) to cardiovascular surgeon (2005). Since 2009, he has held management positions: he successively headed the departments of venous vascular radiosurgery (2009), peripheral arteries (2010), and coronary vessels (2011). In 2021, he combined these areas within the current department. He has the highest qualification category in the specialty of endovascular diagnostics and treatment. He is proficient in the full range of angiography and X-ray surgery techniques,including angioplasty of the coronary, brachiocephalic, renal and peripheral arteries, endovascular aortic prosthetics and transcatheter aortic valve implantation. Since 2015, he has been a member of the board of the Association of Young Interventional Radiologists and Endovascular Specialists (AMIRES), and since 2019, a member of the board of the Russian Scientific Society of Interventional Cardiologists. He regularly participates in Russian and international congresses. He is the author of more than 300 scientific papers, one monograph, and over 30 rationalisation proposals.
