Assessment of acute changes in myocardial function by echocardiography among end-stage kidney disease patients undergoing hemodialysis with high ultrafiltration volume
Abstract
Hemodialysis with high ultrafiltration volume correlates with increased mortality in End-Stage Kidney Disease (ESKD) patients. Acute myocardial function changes remain unclear in them. Aim: Investigate acute myocardial function changes via echocardiography in ESKD patients undergoing hemodialysis with high ultrafiltration volume. A cross-sectional observational study was conducted at Kasturba Medical College, Manipal, India, involving 71 ESKD patients. After regulatory approvals, eligible patients underwent 2D echocardiography before and immediately after hemodialysis. Echo parameters focussed on assessment of systolic and diastolic function, assessment of pulmonary pressures, and strain for LV, RV, and LA. The mean age of the patient was 55.42±12.02 years with an ultrafiltration volume of 16.52±3.8 ml/kg/h.The diastolic function parameters like E/A, E/E’, LAP, E/Vp, and TR jet velocity decreased from 1.48±0.71 to 1.01±0.48,16.64±5.55 to 12.46±4.23, 20.55±5.55 to 16.42±4.23mmHg, 2.50±2.35 to1.45±0.70 and 2.95±0.47 to 2.72±0.46m/s respectively. DT and Vp increased from 162.79±30.32 to 190.72±37.30ms & 49.87±10.19 to 56.75 ± 9.66cm/s respectively. Regarding strain parameters, LV GLS and GRS decreased from17.09±3.00 to 15.64±3.07 % & 32.59±5.96 to 30.66±5.69% respectively. LA PALS, CdS & CtS increased from 21.54±8.00 to 24.79±7.59%, 10.15±4.90 to 11.26±5.21% & 11.37±4.74 to 13.58±4.68% respectively. RVFW strain increased from 26.32±4.02 to 27.92± 4.32%. Patients with preserved EF revealed a decrease in systolic function and an increase in diastolic function immediately after a hemodialysis session. The LV strain values showed a decline after hemodialysis. All three LA and RV free wall strain values were significantly reduced before the hemodialysis and increased after the dialysis session.

Fida Nizar is an astute cardiovascular technologist currently working as a lecturer in cardiac care technology at SRM University, India. She earned an MSc in Echocardiography from Manipal University, India after her BSc program in Cardiovascular Technology. She has won the best presenting author award at the Singapore Heart Foundation Conference 2023.
